The University of Miami/UHealth Department of Pediatrics has an exciting opportunity for a full time Case Worker 2 to work in Miami, FL. This direct service position is responsible for providing maternal child health services to clients (pregnant women and infants) participating in the Healthy Start/CIR CONNECT Programs and related special projects. Duties also include quality assurance and quality improvement activities related to service delivery. Must be willing to conduct home visits with personal vehicles to provide services to clients and interact with other community providers. Position is salaried and requires working evenings and weekends as needed to ensure that job duties are met. Core responsibilities Conducts situational analysis, which may be at home and/or office visits, mentors, and provides technical assistance as needed. Collaborates with the service agencies and program leadership to provide and administer services appropriately. Develops and provides training to family support networks and/or care providers on various topics related to quality early care and education. Maintains and reviews records and files to ensure compliance with project guidelines. Collaborates with all funding sources to ensure the availability of adequate resources. Maintains current knowledge of services available and changes applicable rules and regulations. Attends meetings and training, which may include the Early Learning Coalition, United Way, and the Quality Counts Career Center to enhance developmental potential. Adheres to University and unit-level policies and procedures and safeguards University assets. Department Specific Functions Provide maternal child health services that may include: making initial contacts/phone calls, recruitment, and referral of eligible participants; conducting client assessments; developing individualized care plans to address service needs and risk factors; and providing comprehensive care coordination and wraparound services including parenting education, breastfeeding support, tobacco cessation, inter-conception care education, childbirth education, and psychosocial referral services as needed. Assess client needs for housing and vocational training and make referrals as needed; establish partnerships with local housing providers and employers to further facilitate referrals. For high-risk clients with substance use or substance exposure risk-factors, develop a plan of care to address risks, assess motivation to engage in treatment, and make treatment referrals. Work in conjunction with supervisor and multidisciplinary service team to ensure implementation of coordinated services as related to program participant needs. Provide case management/care coordination referrals to participants when appropriate for the range of maternal child health psychosocial and behavioral health needs. Perform quality assurance tasks, service-related administrative duties and data tasks related to the preparation of programmatic documentation and agency reports and compliance with program performance measures. Performs targeted chart reviews in conjunction with administrative staff. Conduct programmatic educational outreach activities in community settings and facilitate collaborative relationships with community partners. Complete all required client-related service and programmatic written and electronic documentation in compliance with programmatic requirements. Attend all required training and staff meetings and strictly adhere to program protocols. Other organizational/support duties as assigned. Core Qualifications Bachelor’s degree in relevant field required Minimum 3 years of relevant experience required Bilingual, Creole or Spanish preferred Learning Agility: Ability to learn new procedures, technologies, and protocols, and adapt to changing priorities and work demands. Teamwork: Ability to work collaboratively with others and contribute to a team environment. Technical Proficiency: Skilled in using office software, technology, and relevant computer applications. Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.
