Overview We are offering a five‑thousand‑dollar sign‑on‑bonus! Sub‑Acute, Long‑Term Care is a sector of healthcare exploding with potential and work‑life balance flexibility (no on‑call or weekends). We are seeking a full‑time (32‑40 hours per week) provider for our Lebanon Center located in Lebanon, NH. This position can be filled by a nurse practitioner or physician assistant, and new graduates are welcome to apply. The base salary range for this position is $112,320 — $124,800 per year, in addition to our no‑cap incentive compensation bonus program. Responsibilities The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on‑site with the Center Leadership Team for center‑specific goals and priorities. The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient‑specific medical care. The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The Advanced Practice Provider works collaboratively with Center Leadership to achieve AlignMed Partners mission of improving the lives we touch through the delivery of high‑quality health care and everyday compassion. Qualifications Advanced Practice Provider must be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner by graduation from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing, and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations/specialty tracks may be considered based on experience and business need. Physician Assistant must be a graduate from a program accredited by the ARC‑PA, have passed the PA National Certifying Exam (PANCE) to use the PAC designation, and hold PA state licensure. NCCPA certification is required. A minimum of two years of experience working with adult or geriatric populations is preferred. In states where practice is allowed, APRN with an unrestricted professional license and DEA for each state in which they practice.
