Location: Franklin Square Hospital at 9000 Franklin Square Dr

Salary: Offer Based on Years of Experience

Schedule: As available for 8am-5pm weekday coverage in the inpatient unit

Be the Best Hospice Inpatient Visit Physician You Can Be

If you meet these qualifications, we would love to meet you:

• Maintains an active license and DEA in the state where care is being provided.
• Occasional on-call coverage
• (Preferred) Hospice or hospice visit experience

Job Requirements:

• Develop and maintain Plan of Care for each inpatient, including discharge planning discussions with care team, patients, and families
• Participate in prognosis evaluation and certification/recertification
• Sign death certificates as needed
• Conduct daily patient rounds and set consistent rounding schedules with interdisciplinary team
• Ensure proper documentation of eligibility for inpatient care
• Provide education and training to interdisciplinary team and new staff
• Provide after-hours call coverage on days with inpatient visits
