Post Acute Care Physician in Jackson
Overview: Seeking a full-time, quality-focused physician to lead clinical care across skilled nursing facilities in Jackson, Michigan. This role offers high clinical autonomy, dedicated mentorship, and uncapped productivity-based earnings.
Compensation & Schedule
Schedule: Full-time (5 days/week)
Earnings: Estimated $256,000 to $304,000 annually (fee-for-service model with no income ceiling)
Incentives: Additional earning potential via Medicare Shared Savings Program (ACO) participation

Core Responsibilities
Deliver comprehensive clinical evaluations and routine rounding for post-acute patients.
Partner with facility staff, families, and multidisciplinary care teams to optimize patient outcomes.
Spearhead quality improvement initiatives within assigned facilities.
