Job Description

The MDS Coordinator at The Madison Health and Rehab plays a crucial role in managing patient assessments and ensuring compliance with healthcare regulations. This position involves collaborating with a medium-sized interdisciplinary team to oversee data collection, care planning, and quality assurance within a dynamic healthcare environment. The coordinator utilizes Electronic Health Records (EHR) to maintain accurate documentation and supports continuous improvement through regular audits.

Responsibilities

Manage and coordinate Minimum Data Set (MDS) assessments
Collect and analyze resident data for accurate care planning
Review and update resident care plans regularly
Monitor compliance with regulatory standards and prepare for audits
Conduct resident evaluations to support care decisions
Collaborate with interdisciplinary team members (6-15 members) to optimize patient care
Ensure accuracy and completeness of documentation in EHR systems
Support quality assurance initiatives and continuous improvement

Preferred Qualifications

2+ years of experience in healthcare coordination
Associate's Degree in Nursing or related field
Valid Registered Nurse (RN) license
Familiarity with Minimum Data Set (MDS) processes
Proficiency with Electronic Health Records (EHR)
Strong interdisciplinary team collaboration skills
Excellent attention to detail and communication abilities
Effective problem-solving and time management skills
