We are looking for licensed Physical Therapists (PT) to join our growing network in Pima County. What You'll Do Provide Skilled Home Health: Deliver individualized physical or occupational therapy services (evaluations and treatments) in the patient's home environment. Perform Therapeutic Interventions: Focus on functional strengthening, gait training, ADL retraining, balance, and fall risk reduction to help patients reach their independence goals. Report & Collaborate on Patient Progress: Document patient responses to treatment and maintain accurate records to ensure a coordinated approach. Manage Your Caseload: Set your own schedule and visit cadence based on your personal availability and the patient's plan of care. Document Efficiently: Use one simple EMR system for all notes (with tools available to minimize documentation time!).
