Seven Healthcare are seeking an experienced RN specializing in Home Health for a travel assignment in Beaver Dam, WI. This role involves providing quality care to a primarily geriatric population within a post-acute setting, covering both home health and hospice visits. Key Responsibilities of the RN - Home Health

• Provide home care and hospice services for chronic disease management and patient/family education
• Manage a caseload of 5-6 patients per day, including routine and OASIS visits
• Participate in weekly IDT meetings and monthly all staff meetings
• Utilize skills in pain and symptom management, end-of-life care, and complex wound care
• Perform NPWT, ostomy care, central line care, and IV administration

Requirements of the RN - Home Health

• Active RN license with travel experience
• Certification in BLS is required
• Proven experience in chronic disease management and hospice care
• Proficiency in Cerner charting system
• Skills in critical thinking and complex wound care management
