Job Overview An Infusion Nurse I is a licensed registered nurse who provides basic direct patient care to patients at home or in an alternate infusion site, ensuring patient safety and continuity of care under a physician’s plan of care. The role is supervised closely by a designated supervisor. Additional Role: HHA Administrator Responsibilities (Including California Requirements) In addition to clinical infusion nursing duties, this role will serve as the designated Home Health Agency (HHA) Administrator (or Alternate Administrator) for the assigned location, ensuring regulatory compliance, operational readiness, and survey preparedness. Key Administrator Responsibilities Regulatory & Survey Readiness Serve as primary point of contact for Regulatory & Accreditation communications and state/Joint Commission survey activities. Ensure availability within one (1) hour for unannounced surveys or delegate to the Alternate Administrator. Coordinate on‑site readiness with appropriate departments to include facility access, internet, and required postings. Participate in mock survey activities as requested by the Regulatory and Accreditation department. Operational Oversight Maintain compliance with all site‑level requirements, including: Active licensure postings (business license, HHA license, accreditation certificate) Required federal/state labor law postings On‑site company communication device and emergency/basic supply kit Maintain accurate administrative oversight while working within nursing leadership structure. Emergency Preparedness Coordinate with nursing leadership to activate the Emergency Operations Plan (EOP) during emergencies. Participate in emergency drills, documentation, and after‑action reporting. Compliance & Reporting Requirements Complete and submit quarterly HHA inspections and ensure follow‑up actions. Conduct and document annual fire drills and support After‑Action Reports (AARs) with support from the Regulatory and Accreditation Coordinator. Maintain familiarity with key compliance documents (EOP/EMP, inspections, regulatory files). Leadership & Coordination Ensure Administrator and Alternate Administrator designations remain current and compliant. Collaborate with Nurse Managers, Senior Nurse Managers, and Regulatory & Accreditation teams. California‑Specific Administrator Requirements (Title22 & CDPH Alignment) For roles supporting California‑licensed Home Health Agencies, the Administrator must meet state‑specific qualifications and regulatory expectations. Minimum Qualifications Qualified as a licensed Registered Nurse (RN), physician, or hold a bachelor’s degree (or higher). Have at least one (1) year of supervisory or administrative experience in healthcare or a related field. Experience & Oversight Requirements Demonstrated experience in health service administration, including oversight of personnel, operations, and compliance activities. Capable of directing agency operations in accordance with California Code of Regulations, Title22 governing Home Health Agencies. Background & Clearance Requirements Must successfully complete criminal background clearance via Live Scan fingerprinting prior to performing duties. Must meet “good character” requirements as defined by California Health & Safety Code for licensed agencies. Role Flexibility Disclaimer This position includes dual clinical and administrative responsibilities. Administrator duties, including California‑specific requirements, may evolve based on regulatory updates, CDPH guidance, accreditation standards, and organizational needs. Job Responsibilities Initiate, develop, and implement basic nursing plan of care treatments, evaluating patient progress toward goals. Participate in the provision of direct patient care, administer medications, and educate patients and families/caregivers. Assess patient needs and physical status at each visit through health data access and patient interview. Re‑evaluate patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtain and clarify physician orders for plan of treatment revisions, inform physician promptly of significant changes in patient’s condition, and provide written summary to physician. Provide effective teaching using a patient‑centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals. Effectively and timely communicate with Naven Health’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepare clinical documentation in real time during visits and submit to the nursing department to comply with established timelines for billing optimization. Under close supervision, coordinate discharge planning and prepare discharge summaries with patient instructions and fully report patient care needs, progress, and goals when transferring care. Demonstrate compliance with agency operations, Naven Health’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximize work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicate patient care provided, and follow assigned visit schedule. Respond promptly and appropriately to patient requests. Initiate emergency procedures as necessary. Accept accountability for own practice through ethical and professional conduct. Follow established programs and practice within policies and procedures reflective of Naven Health’s mission, values, and objectives. Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary Naven Health information, including adherence to HIPAA regulations. Speak knowledgeably about Naven Health’s scope of services and effectively instruct patients about related plan of care. Attend and complete required training modules, in‑services, and continuing education to maintain competency and professional licensure for demonstrated knowledge regarding the care and management of patients in the home and/or alternate care settings. Effectively provide oversight and coordination of paraprofessionals in the home setting and may act as the Supervisor in the absence of the Supervisor or Nurse Manager as applicable in accordance with state and federal regulations. Participate in multidisciplinary team conferences and provide precepting, training, and mentoring to other nurses for orientation, onboarding, and supervisory activities as assigned. Participate in nursing on‑call responsibilities as applicable. Perform other related duties as directed by supervisor. Supervisory Responsibilities No supervisory responsibilities (no hiring, performance reviews, scheduling, etc.). Basic Education and/or Experience Requirements Active and unrestricted RN license in the state of practice. Minimum of 0–2 years of infusion nursing experience required. Current CPR certification required (an in‑person class is required per company policy). Basic Qualifications Demonstrated competency in patient care standards required for safe delivery of services and infusion skill sets applicable to agency programs and service needs. Advanced certification and training as applicable. Required licensure to operate a motor vehicle in the state of practice with access to a vehicle for business travel with proof of liability insurance. Basic knowledge of computer operating systems and software applications with ability to self‑evaluate for annual appraisal and jointly set professional growth goals with nurse manager; apply knowledge in the effective use of nursing technology tools to communicate and document care provided. Physical Demand Requirements Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required, including ability to see up close and from a distance, color and peripheral vision, depth perception and ability to adjust focus. Team members in this job classification have the likelihood of occupational exposure to blood, body fluids and other potentially infectious materials. Possible exposure to hazardous substances, injury from needles, other sharps, fumes, chemicals, humidity, cold, heat, adverse weather elements, animals, secondhand smoke/vape and unpredictable home environments. Safety requirements include closed‑toe shoes, facemask, goggles, gown and/or gloves and functioning device for communication in both routine and emergency situations. Travel Requirements Willing to travel 100% of the time for business purposes. Preferred Qualifications & Interests Bachelor of Science in Nursing as granted by an accredited school of nursing preferred. 1 year of previous experience as an RN in a home health or acute care setting.
