Hospice RN Clinical Liaison

Heartland Hospice

Heartland Hospice, part of ProMedica, is seeking an RN Clinical Liaison to coordinate hospice referrals and admissions for patients with life-limiting illnesses. The RN Clinical Liaison conducts clinical assessments, explains hospice services, and facilitates smooth transitions from hospitals, facilities, and homes into hospice care. This role collaborates closely with physicians, discharge planners, and the interdisciplinary hospice team to develop care plans focused on comfort, dignity, and symptom management. The RN also educates patients and families, ensures regulatory and documentation compliance, and represents Heartland Hospice with compassion and professionalism in the community.
