Minimum Requirements
Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree approved by the APTA.
Current state license as a Physical Therapist in Kentucky required to start the assignment.
Proof of current CPR certification required.
Current driver’s license, reliable transportation, and vehicle insurance required.
1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting required.
Experience with durable medical equipment and ordering rehabilitative equipment required.
Ability to perform patient lifting, transfers, prolonged standing and walking, and manual dexterity consistent with home health duties.
Ability to work a flexible schedule and travel locally required.
Must read, write, and speak fluent English.
Previous home care experience preferred.
