LPN Caregiver

The Licensed Practical Nurse (LPN) provides skilled nursing care and personal support services to a patient in the home setting with complex medical needs, including a tracheostomy, ventilator, and PEG tube. Working under the supervision of a Registered Nurse, the LPN delivers high-quality clinical care while assisting with activities of daily living to promote the patient's comfort, safety, dignity, and overall well-being.

Location: Ambler / Blue Bell, PA Area

Available Shifts:

• Saturday & Sunday
• 7:00 AM - 7:00 PM OR
• 7:00 PM - 7:00 AM

Perfect for nurses seeking consistent weekend shifts while providing meaningful, one-on-one patient care.

Provide skilled nursing care for a patient with a tracheostomy, ventilator, and PEG tube.

Monitor and assess the patient's physical, emotional, and clinical condition.

Execute the patient care plan and communicate effectively with the RN Case Manager.

Administer medications and treatments as prescribed and authorized.

Assist with tracheostomy care, ventilator monitoring, and enteral feeding support.

Provide personal care, including bathing, grooming, dressing, toileting, and hygiene assistance.

Assist with mobility, positioning, transfers, and use of medical equipment, including a Hoyer Lift.

Prepare meals and assist with eating and hydration as appropriate.

Follow nutritional guidelines and care instructions established for the patient.

Perform light housekeeping duties within the patient's living area.

Help maintain a clean, safe, and comfortable home environment.

Accompany the patient to approved medical appointments and community outings as authorized.

Observe and report changes in condition promptly to the care team.

Complete accurate and timely documentation using the Agency-approved Electronic Visit Verification system.

Demonstrate accountability to assigned schedules and communicate scheduling needs to the Staffing Coordinator.
