Responsibilities

Provide direct patient care under the supervision of our experienced nursing staff, including administering medications, performing wound care, and assisting with ADLs. Develop and implement individualized care plans in collaboration with the interdisciplinary team. Monitor and report patient progress, including identifying and addressing any changes or complications. Maintain accurate and detailed records of patient care and treatment. Participate in quality improvement initiatives to improve patient outcomes and satisfaction.

Requirements

Currently licensed as a Practical Nurse in the state of Pennsylvania. Ability to work effectively in a fast-paced environment with a focus on patient care. Strong communication and interpersonal skills. Basic computer skills and familiarity with electronic medical records.
