Key Responsibilities\n

• Provide crisis intervention and counseling services, addressing complex emotional and psychological issues and helping clients navigate end-of-life decisions.

• Educate clients and families on hospice services, community resources, and coping strategies to manage grief and loss.

• Document all interactions and assessments accurately in our electronic health record (EHR) system, ensuring compliance with state regulations and organizational policies.

• Stay informed about advancements in social work practices and regulations in hospice care to continually provide the best support to clients.

Patient Population / Client Focus\n

Your expertise will primarily serve hospice patients, who face complex emotional and psychological challenges during life's most critical phase. You will work closely with patients who may be grappling with terminal illnesses, as well as their families, focusing on enhancing their emotional well-being and facilitating meaningful conversations about their wishes and end-of-life care.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution.

• Current and active Licensed Clinical Social Worker (LCSW) license in New Jersey.

• Proficiency in conducting diagnostic evaluations and clinical assessments in a healthcare setting.

• Exceptional communication skills, both verbal and written, with the ability to foster relationships and build trust with clients and their families.

Compensation and Benefits\n
