Nurse Practitioner | Palliative CareA growing palliative care organization is seeking an experienced Nurse Practitioner to provide compassionate, high-quality care to patients with serious illnesses and complex chronic conditions throughout Ocean County and surrounding areas.This role offers the opportunity to work within a collaborative interdisciplinary team while making a meaningful impact on patients and families through comprehensive symptom management, advance care planning, and patient-centered care.Position Highlights• Flexible schedule • Meaningful work focused on improving quality of life • Collaborative interdisciplinary care team • Opportunity to build lasting patient and family relationships • Supportive clinical environment with professional growth opportunitiesResponsibilities• Conduct comprehensive palliative care assessments and follow-up visits • Manage patients with serious illness and complex chronic conditions • Facilitate goals-of-care conversations and advance care planning discussions • Develop and implement individualized care plans • Collaborate with physicians, specialists, hospitals, skilled nursing facilities, and community providers • Provide symptom management and patient/family education • Document patient encounters accurately and in a timely manner • Participate in interdisciplinary team meetings and care coordination activitiesQualifications• Active New Jersey Nurse Practitioner license • National board certification and DEA registration • Minimum 2 years of Nurse Practitioner experience preferred • Experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or chronic disease management preferred • Strong communication and interpersonal skills • Ability to work independently while collaborating within a multidisciplinary teamPreferred• Hospice & Palliative Care certification (ACHPN)• Experience with home-based care, telehealth, or value-based care models • Bilingual skills are a plusOcean County, New JerseyLocated along New Jersey's beautiful Jersey Shore, Ocean County offers a unique blend of coastal living, vibrant communities, and excellent recreational opportunities.
