Senior Living Physical Therapist

BAYADA Home Health Care is currently seeking an experienced Physical Therapist, PT for a full‑time opportunity performing outpatient home health visits for our senior‑living office located in Londonderry, New Hampshire . This office serves adult and geriatric clients on a per‑visit basis in the Londonderry and Seacoast territories. Up to $15,000.00 bonus available for full‑time 4 or 5 day schedules! One year of prior clinical experience as a licensed Physical Therapist is required. As a home‑care Physical Therapist, you will be an integral member of a multidisciplinary health‑care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to remain at home. Responsibilities Make outpatient visits to clients in designated geographic territories. Perform diagnostic tests and measurements (mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments). Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance training, and other interventions. Continually assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan, as appropriate. Educate and instruct clients, family members, or other representatives in rehabilitative care and activities for health, safety, and independent living. Accurately document observations, interventions, and evaluations of client care using a state‑of‑the‑art touch‑pad tablet. Qualifications A current New Hampshire Physical Therapist – PT license. Minimum one year of recent work experience as a Physical Therapist – PT. Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
