Experienced Physical Therapist

Provide home health physical therapy services to adult and geriatric patients in their homes. Conduct initial evaluations, reassessments, OASIS documentation, recertifications, and discharge planning. Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence. Manage an average of 4-6 patient visits per day while maintaining productivity standards. Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers to ensure continuity of care. Accurately document patient care using Homecare Homebase (HCHB). Participate in weekly case conferences and coordinate care plans as needed. Travel within the assigned coverage territory, with occasional support in nearby service areas.

Active California Physical Therapist (PT) License. Minimum 1-2 years of Physical Therapy experience, preferably in Home Health. Home Health experience is REQUIRED. Strong experience with OASIS documentation (No exceptions). Experience with Homecare Homebase (HCHB) preferred. Candidates without HCHB may be considered at a reduced rate during the initial training period. Valid driver's license, reliable transportation, and ability to travel within a 60-mile service radius. Excellent communication, documentation, and patient care skills.

Excellent Pay: Earn up to $2,699/week plus IRS mileage reimbursement at $0.725/mile. Work-Life Balance: Enjoy a Monday-Friday schedule (8 AM - 5 PM) with no regular weekends. Guaranteed Hours: Consistent 40 hours/week throughout the 13-week contract.
