Therapeutic Intervention. Therapeutic Interventions offers Foster Care and Foster-to-Adopt services in Middle and East Tennessee and Memphis/Shelby County. Therapeutic Interventions has been providing services to children, youth, and families for over 28 years. At Therapeutic Interventions, we help children and families by helping them meet their basic needs of food, shelter, love, emotional support, consistency, and structure. We offer continuous development and growth are critical in providing services to children and families.

Summary:

The Therapeutic Foster Care Case Manager provides professional case management services for children and families, in compliance with all regulatory guidelines, including the Department of Children’s Services (DCS) and Therapeutic Interventions regulation, policy, and procedure. The Therapeutic Foster Care Case Manager is empowered to observe, document, coordinate and provide services for children, families, and therapeutic foster parents, to support the goals of child safety, security, and expedient permanency.

Essential Duties & Responsibilities :

• Provide professional and personalized case management services, including observing, monitoring, coordinating, and documenting client progress and/or issues.

• Follow and implement the agency’s adopted evidence-based case management model.

• Create, secure, and maintain required case documents in child and foster parent records.

• Monitor daily notes and medication logs for foster homes.

• Ensure all necessary appointments are monitored and completed to meet child needs, in compliance with regulatory guidelines.

• Travel regularly to facilitate family time visits and appointments.

• Serve as a liaison with public and private agencies in support of children and families.

• Deliver timely, accurate, and complete reports, treatment plans, monthly summaries, weekly case notes, and other required documentation.

• Ensure child case records are accurate, complete, and compliant with agency standards.

• Routinely staff cases with Regional Director regarding child and family treatment plan progress and implementation, placement transfers, support services, and incidents.

• Participate regularly in monthly and/or quarterly meetings and may be invited to serve on various committees.

• Continuously collect data regarding child outcomes, permanence, length of stay, discharges, transfers and adoptions, and all other data related to client case management, ensuring accurate and complete data is uploaded or entered in Extended Reach/any related DCS databases in a timely fashion.

• Follow all TII policies and procedures and DCS contract requirements, DCS licensing requirements, and COA standards

• Standard office hours are Monday through Friday, 8:00 a.m. to 5:00 p.m.; however, this position requires participation in a mandatory on-call rotation to provide coverage during evenings, nights, and weekends.

• Employees are expected to fulfill planned responsibilities (e.g., meetings, transports, admissions, or other assigned duties) that may occur outside of standard business hours, including evenings and weekends.

• This role may involve unplanned or urgent work requirements that necessitate adjusting or rescheduling the workday, when not on approved leave, in order to support team operations outside of the on-call rotation.

• In the event that overtime is accrued, employees may utilize flex time—defined as adjusting work hours within the same (7 day) workweek to offset additional hours worked, subject to operational needs. Flex time may be taken on any day within the same week, excluding Fridays.

• Alternatively, overtime must be pre-approved by a supervisor if it cannot be accommodated through flex time.

• Other related duties, as assigned.

Qualifications: A bachelor’s degree from an accredited college or university is required, along with a minimum one (1) year previous experience working with children or one (1) year casework experience. A master’s degree in the Social Services field from an accredited college or university is preferred. Demonstrated, proficiency in Microsoft Office Suite is required. Demonstrated ability to safeguard highly confidential clients and foster parent information is also required, along with professional communication, interpersonal skills, outstanding organizational and planning skills.

Certificates, Licenses, Registrations: Valid driver’s license, proof of automobile insurance, successful completion of pre-employment and background checks, including motor vehicle record check(s)

Physical Demands : This position has the flexibility of working from home but requires frequent travel to foster homes, family homes, meetings, and appointments, as well as travelling to the office to use office equipment as needed to perform job duties. This position has responsibilities for transporting children on a frequent basis. The Therapeutic Foster Care Case Manager will routinely interact and communicate with children and families, each Child and Family Team, and public and private agency contacts, to ensure child safety and security while facilitating expedient child permanency and therefore, is regularly required to read, write, type, and interpret text, operate an automobile for transporting youth, and sit, stand, walk, talk, hear, reach, bend, and lift up to 20 pounds.

Performance Evaluation: Employees are evaluated on an annual basis. Performance Reviews are completed electronically by the employee’s direct supervisor then reviewed and approved by regional or program leadership. Employees are given the opportunity to review their performance evaluation with their supervisor and will be required to acknowledge receipt of the annual review electronically.
