RN Hospice Care Manager — Lead Patient Care & Quality
Optum
Baptist Trinity Hospice, part of the LHC Group within the Optum family, seeks a Patient Care Manager to support executive leadership in delivering safe, efficient patient care. The role requires occasional travel to patients in the community and oversight of an interdisciplinary hospice team. Primary responsibilities include regulatory compliance, quality and safety leadership, staff education, and effective problem resolution.
