This role plays a critical part in supporting providers, ensuring clinic efficiency, and delivering high-quality patient-centered care.Key Responsibilities & Strengths:Accurate and timely patient triage, including assessment of walk-ins and urgent concernsPerformance and documentation of point-of-care testing with appropriate follow-upProcessing medication refills in compliance with clinical protocolsPrompt response to patient and staff emergencies with appropriate escalationDelivery of comprehensive patient education to reinforce care plansThe ideal candidate is organized, proactive, and comfortable balancing clinical and administrative responsibilities. Critical thinking, compassion, autonomous, and efficient communication skills and the ability to prioritize in a high-volume outpatient environment are essential.Hours: Mon- Friday 8AM-5PMCollaborates with interdisciplinary team in the delivery of patient care, under the direction of a Registered Nurse, utilizing the nursing process.Minimum Qualifications:Education: Graduate from an accredited school of vocational nursing or licensed practical nursingLicenses/Certifications:Current State of Texas license or temporary/compact license to practice as a Licensed Vocational Nurse (LVN);Certified in Basic Life SupportAdditional certifications and language skills may be required based on discipline and/or nursing unit requirementsExperience / Knowledge / Skills:Minimum of (3) three years prior LVN experience preferably in acute care settingPrincipal Accountabilities:Delivers safe and appropriate care to patients, according to the nursing skills inventory and the age and needs of patient/population served, within the scope of licensure.The LVN collaborates with the interdisciplinary team in the delivery of patient care; supporting and collaborating with the Registered Nurse in providing direction to the healthcare team.Participates in a systemic, interdisciplinary, and ongoing evaluation of programs, process improvement and desired client-centered outcomes.Considers the diverse needs of each patient through the entire continuum of care, providing and ensuring the highest level of service according to established service standards, optimizing the patient experience.Adheres to all regulatory and Texas Board of Nursing standards. Performs other duties as assigned, within the scope of practice and clinical competency.Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences.
