Personal Care Aide / Home Health Aide

Job Description

SagePoint Healthcare LLC is seeking a reliable and compassionate caregiver/home health aide for a client in the Springfield, OH 45502 area .

This is a part-time position offering 24 hours per week with a consistent schedule:

Thursday, Friday, and Saturday: 12:00 PM – 8:00 PM

Responsibilities

Assist with activities of daily living and personal care as authorized in the client’s service plan
Provide homemaking and light housekeeping assistance
Assist with meal preparation
Provide companionship and support
Help maintain a safe and comfortable home environment
Follow the client’s care/service plan and SagePoint policies
Accurately document services provided and comply with EVV requirements
Report changes in the client’s condition or concerns to the appropriate supervisor

Qualifications

Dependable, compassionate, and professional
Ability to follow a service plan and agency procedures
Reliable transportation to the client’s home
Must successfully complete all required pre-employment screening, background checks, training, and onboarding requirements before assignment
Previous caregiving, home care, STNA, HHA, or similar experience is preferred
