Bilingual Field-Based Community Health Navigator II
ADP, Inc.
MedZed is seeking a Community Health Navigator II to provide field-based case management for higher-acuity Medicaid members in LA County. The role combines door-to-door outreach, assessments (PHQ-9 included), and care plan development with med plans and community resources.
