Case Manager CBI is the premiere non‑profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, providing prevention, education, and treatment using nationally recognized evidence‑based models. Job Summary The Case Manager provides case management services in either the community or office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating, and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. The case manager embraces the recovery philosophy and promotes a safe recovery environment for clients. Skills / Requirements High school diploma or GED required; associate’s degree or higher in a field related to behavioral health preferred. Minimum of six months recovery from substance use and/or mental health disorders preferred. 1–3 years of experience working with the GMH/SU population required. 1–3 years of experience in a behavioral health position providing outreach and engagement activities required. 1–3 years of experience in a medical setting preferred. Valid identification required. Current AZ Driver’s License (valid and in good standing) preferred. 39‑month MVR preferred. Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures. Arizona Fingerprint Clearance card. Benefits Generous PTO accrual (5 weeks). Hospital indemnity / Critical Illness coverage. Pet Insurance.
