Physical Therapist — Outpatient, Chandler, AZ

Seeking a licensed Physical Therapist to provide outpatient orthopedic care in Chandler, AZ. The clinician will evaluate patients, develop and implement treatment plans, and document care according to regulatory and clinic standards. Responsibilities include patient and family education, supervising discipline-specific therapy services for assigned patients, participating in performance improvement activities, and contributing to a positive learning environment for students when assigned. Clinician will follow the organization’s safety practices and professional standards.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Arizona State physical therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience providing outpatient orthopedic physical therapy.
• Demonstrated ability to complete comprehensive evaluations, develop functional treatment plans, and document care in compliance with state, federal, and payer requirements.
• Ability to participate in student training and provide supervised learning experiences, when applicable.
• Adherence to professional state practice act and maintenance of required continuing education and licensure.

Preferred Qualifications

• Prior travel or contract experience in outpatient therapy settings.
• Experience with charge entry, letters of medical necessity, and functional quality indicator assessment.

Assignment Details

• Shift pattern: 5x8.
