Synergy Care is hiring a full-time Physical Therapist for an outpatient clinic serving an AL/IL community in Fort Worth, TX!
We are looking for a team-oriented clinician who is passionate about providing quality patient care and making a difference in the lives of others. Job Responsibilities:•Evaluate patients and develop individualized treatment plans
•Provide ongoing treatment and discharge planning
•Supervise assistants and aides
•Participate in case management
•Deliver high-quality occupational therapy services in accordance with state regulations and Synergy Care guidelines
•Represent Synergy Care's commitment to excellence in every patient and facility interaction Benefits Include:
