Job Details

• Position Description: Hospice Float Registered Nurse responsible for patient care in a home health/hospice environment, with up to six patient visits per day.
• Certifications & Experience: BLS certification, at least 1 year of skilled hospice or home health experience, proficiency with HCHB and triage experience required.
• Location Requirements: Local candidates within a 50-75 mile radius considered. Pending license accepted.
• Schedule: 10-hour night shifts, 10:00 PM – 8:30 AM, minimum 30 hours per week.
• Responsibilities: Providing comprehensive hospice care, including advance directives, death pronouncement, family support, medication management, and patient education.
• Additional Information: Must have own transportation; facility arranges patient grouping to minimize drive time; occasional maximum radius of 50+ miles; home care home base experience required.
• Submission Requirements: Candidates must include responses regarding prior work at a Providence facility, two references covering one year within the last three years, and confirmation of at least one year of relevant experience.
