As an employer, we seek to provide an environment of growth and development for our employees that is consistent with recognizing the rights, needs, and wants of all people.Purpose:To provide support and assistance to individuals with intellectual and developmental disabilities to enable them to live valued lives in the community.Qualifications:Must successfully complete the required pre-employment Background and Drug ScreeningsMust possess a valid Driver's License with all necessary endorsements and have a minimum of 3 years of driving experience.Must possess advanced computer skillsMust possess a High School Diploma or equivalent, Bachelor’s Degree preferredExperience working in the field of intellectual/developmental disabilities preferredMinimum one year of experience in writing program plans, case notes or other relevant documentation requiredEssential Functions:Responsible for the development, implementation and coordination of individual’s program plansDevelop, implement and monitor individualized training programs to meet identified priority training needsEnsures all plans are person centered and reflect the individual’s hopes and dreams through active participation wherever possibleEnsure all documentation is accurate and complete according to regulatory requirementsCommunicate with all team members, including family, guardians and support coordinators to ensure that the individual’s needs are being addressedPerforms other duties as assignedThis position will remain open until filled.
