Registered Nurse - Case Management

Location: Fort Pierce, Florida

Contract Type:  Travel Contract

Shift Type:  Monday-Friday, 8:00 AM - 4:30 PM, rotating every other weekend

Hours per Week:  40

Candidate Pay Rate: $2000- $2150 Weekly

Roles & Responsibilities

• Coordinate seamless care transitions as a vital RN Case Manager
• Collaborate within a dynamic RN/SW/CMA triad model to support patients
• Perform utilization review using Interqual criteria for initial and continued stay reviews
• Develop comprehensive discharge plans that empower patients and families
• Adapt quickly to high-acuity units and float across the facility as needed
• Leverage tools like Meditech, Midas, and Outlook to streamline case management workflows

Qualifications

• Active RN license in Florida or Compact state
• 2-4 years of RN Case Management experience in an acute care setting
• Comfortable working in a fast-paced, high-acuity Level II trauma environment
• Previous HCA experience is a plus, along with familiarity using Meditech, Midas, and Outlook

Knowledge & Skills

• Strong case management and discharge planning expertise
• Proficiency with utilization review processes and Interqual guidelines
• Ability to float across multiple units and adapt to changing priorities
• Excellent communication and collaboration within multidisciplinary teams
