Trabajador de Salud Comunitaria I - Recetas de Alimentos (Flotante)

Responsabilidades El Trabajador de Salud Comunitaria será un miembro integral del equipo de atención interdisciplinaria de Harris Health, colaborando para ayudar a los pacientes a mejorar su salud de manera holística. El TSC trabajará directamente con los pacientes para evaluar las necesidades en todos los aspectos de sus vidas, incluidos los desafíos económicos y sociales junto a sus necesidades de salud física y mental. El TSC será la persona de contacto para un grupo de pacientes, disponible y accesible para brindar asistencia. Navegación de pacientes, gestión de cuidados o cualquier tipo de prestación de servicios directos a pacientes en un entorno de salud comunitaria/outreach, agencias de recursos sociales y/o de atención médica. Calificaciones y Habilidades Se prefieren habilidades bilingües que incluyan español/inglés e inglés/vietnamita. Las habilidades de comunicación requeridas son verbales por encima del promedio con un gran contacto público, así como escritura/correspondencia. Las competencias necesarias incluyen habilidades con computadoras personales.
