Role: Physical Therapist responsible for providing compassionate, evidence-based care to home health and hospice patients. Responsibilities Assess patients' functional status and develop individualized treatment plans. Help patients regain mobility, manage pain, and maintain safety at home. Collaborate with an interdisciplinary team to ensure comprehensive care. Educate patients and caregivers on treatment and health management. Document clinical progress accurately and thoroughly. Qualifications & Skills Strong clinical knowledge in physical therapy and patient care. Excellent communication and teamwork abilities. Ability to develop personalized treatment strategies. Commitment to patient-centered, evidence-based practices. Key Duties: Deliver high-quality therapy services in a home setting, ensuring patient safety and progress, while working closely with healthcare teams and educating patients and caregivers. #J-18808-Ljbffr
