Care Specialist

Our busy home health agency is rapidly growing, and we’re seeking an experienced home health aide to assist us in providing superior in-home health care to clients. Our ideal candidate will have excellent organization and communication skills, and 1-3 years of experience working as an HHA or caregiver. You’ll assist our patients with companionship, basic medical care, light housekeeping duties, personal care, and hygiene tasks so that they are empowered to live happily at home. Responsibilities: • Maintain psychological and emotional support for patients and family members - provide quality companionship • Complete basic medical tasks such as taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices • Stay on top of light housekeeping tasks including but not limited to laundry, bed making, vacuuming, etc. • Implement care plans as directed by the case manager, and keep updated and accurate care records • Assist patients in completing personal hygiene tasks like bathing, grooming, dressing, toileting, shaving, etc. Administer Medications: Dispense medications as directed by the Registered Nurse (RN), maintaining an annual error rate of no more than one mistake.Resident Response: Attend to resident call lights within 10 minutes to ensure responsive and attentive care.Light Housekeeping: Perform regular light housekeeping in resident rooms at least once weekly to promote a clean and comfortable living space.Service Charting: Accurately document all services provided without missing tasks, ensuring compliance with company standards.Assistance with ADLs: Support residents in Activities of Daily Living (ADLs) according to each individual’s Client Service Plan, with a zero-tolerance policy for missed tasks.
