Responsibilities The Physical Therapist primarily provides outpatient rehabilitation services in an in‑home setting for older adults, in private residences or independent living facilities. Evaluates, develops, and delivers physical therapy programs to facilitate rehabilitation, including assessment and planning of individualized therapy programs. Provides referrals to other disciplines and agencies; consults with the rehabilitation team to select appropriate therapeutic regimens. Supervises PT assistants, temporary licensees, students, and support personnel as required. Maintains adherence to treatment schedules, documentation, billing, and meetings. Promotes a safe environment and enforces fire and safety hazard elimination. Orders and maintains supplies and equipment, identifying and reporting repairs. Directs patient services, ensuring high‑quality, compassionate care. Embodies functional training, manual therapy, airway clearance techniques, integumentary repair, modalities, health and wellness programming, chronic disease education, fall risk assessment, and case management. Designs and applies adaptive devices, trains patients in their use, and selects appropriate activities. Instructs patients and caregivers in mobility skills; evaluates progress, attitudes, and behavior. Performs routine re‑examination as necessary to modify treatment plans. Incorporates health and wellness programming into treatment plans. Conducts home and community mobility assessments and provides recommendations for safe mobility. Qualifications Valid Physical Therapy license in the state(s) of practice or proof of license eligibility. Master’s degree in Physical Therapy, or a Master’s in a related field plus a Bachelor’s in Physical Therapy, or a Bachelor’s in Physical Therapy plus five years of progressively responsible PT experience. U.S. equivalent evaluation required for degrees earned outside the U.S. Valid driver’s license or reliable independent transportation to patients’ residences.
