- Assess residents' speech, language, and swallowing abilities to develop individualized treatment plans.
- Implement therapy interventions to enhance communication skills and dietary safety.
- Document patient progress accurately and adjust treatment plans as needed.
- Educate residents and their families on communication strategies and exercises.
- Comply with all regulatory and facility standards to ensure quality care.
- Participate in interdisciplinary team meetings to coordinate care delivery.
- Valid state licensure as a Speech Language Pathologist.
- Recent experience working with adult populations in a skilled nursing facility preferred.
- Strong evaluation and treatment planning skills.
- Excellent communication and documentation abilities.
- Ability to work independently and as part of a team.
- BLS certification is required or must be obtained.
