About the Role: We're seeking a certified and experienced Home Health Aide to provide personalized care and support to our patients. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver exceptional care and promote independence, comfort, and well-being. Responsibilities: Provide personal care assistance to patients, including bathing, dressing, grooming, and toileting Assist with daily living activities, such as meal preparation and medication management Support patients with mobility and ambulation, including transfers and exercises Monitor patients' health status and report changes to the supervising nurse Collaborate with the healthcare team to develop and implement individualized care plans Maintain accurate and confidential records of patient care and communication Requirements: Current certification as a Certified Home Health Aide (CHHA) High school diploma or equivalent required Ability to lift 50 pounds and stand for long periods Strong communication and interpersonal skills Compassionate and patient-centered approach to care Why Join TruHeart Hospice - Norman?
