Physical Therapist - Travel Contract

Minimum Requirements

• Current State License as a Physical Therapist in South Carolina or compact/state licensure required prior to start. (Client requires current state license.)
• Bachelor of Science, Master, or Doctoral degree in Physical Therapy from an APTA-approved program. (Bachelor of Science in Physical Therapy required by client.)
• Proof of current CPR certification. (Client requires CPR certification.)
• Current driver’s license, reliable transportation, and vehicle insurance. (Client requires reliable transportation and insurance.)
• 1-2 years of experience as a Physical Therapist in acute care or rehabilitation settings. (Client requires 1-2 years of relevant experience.)
• Ability to perform heavy lifting, bending, pulling, pushing, prolonged standing and walking as required for home health visits. (Client physical demands specified.)
• Ability to work a flexible schedule and travel locally. (Client requires local travel capability.)
• Must read, write and speak fluent English. (Client requirement.)
• Preferred: Previous home care experience.

Assignment Details

• Facility Type: Home Health.
• Assignment Type: Travel / Temporary Contractor.
• Must plan, implement, and evaluate patient care plans and document Plans of Care, progress notes, and phone orders per agency standards.
• Deliver therapy for neuromuscular, neurological, and orthopedic diagnoses and instruct patients/families on transfers and equipment use.
• May coordinate with interdisciplinary team and supervise other team members when applicable.
