Situated in the safe, historical Tenleytown neighborhood, PIW is one mile from the Tenleytown Metro stop and within walking distance to local cafes and restaurants.The Lead Patient Care Coordinator (LPCC) plays a crucial role in supporting individuals admitted for high-risk opioid use by delivering personalized, compassionate care from admission through aftercare planning. Serving as both a guide and advocate, the LPCC facilitates individual and group sessions to help patients identify their needs, overcome challenges, and build effective recovery plans. Leveraging strong knowledge of community-based resources, they coordinate seamless access to ongoing treatment and support services.
