MDS Coordinator, RN

We are seeking an experienced Registered Nurse (RN) to join our skilled nursing team as an MDS Coordinator . This is an excellent opportunity for a detail-oriented clinician who is passionate about resident care, accurate reimbursement, regulatory compliance, and interdisciplinary collaboration.

Responsibilities

Coordinate and complete the MDS assessment process in accordance with CMS and state regulations.
Ensure timely and accurate completion of MDS assessments and care area assessments (CAAs).
Collaborate with the interdisciplinary team to develop and update individualized care plans.
Maximize Medicare, Medicaid, and managed care reimbursement through accurate documentation and coding.
Review clinical documentation for accuracy and completeness.
Monitor Quality Measures (QMs) and assist with quality improvement initiatives.
Maintain compliance with federal, state, and facility regulations.
Participate in survey preparation and respond to audit requests.
Educate nursing staff on documentation requirements and MDS processes.

Qualifications

Current Registered Nurse (RN) license in Connecticut (required).
Minimum 2 years of MDS Coordinator experience in a skilled nursing or long-term care setting.
Strong working knowledge of RAI Manual, MDS 3.0, PDPM, Medicare, Medicaid, and CMS regulations.
Experience with PointClickCare or other electronic health record systems preferred.
RAC-CT certification is a plus.
Excellent organizational, analytical, and communication skills.
Ability to manage multiple deadlines while maintaining accuracy.
