Post-Acute Care Physician (Internal Medicine / Family Medicine) Location: Los Angeles County, CA Position Type: Full-Time Schedule: Monday–Friday Compensation: $260,000–$325,000 annually Leadership stipend: $30,000–$40,000 annually for APP oversight (if separate from productivity) wRVU productivity bonus: Paid above an established threshold Comprehensive benefits including health, dental, vision, Retirement, PTO, CME, malpractice with tail coverage, and mileage reimbursement. Position Summary The Post-Acute Care Physician is responsible for providing comprehensive medical care to patients residing in skilled nursing facilities and assisted living communities. This is a mobile position , requiring travel between assigned facilities to conduct patient rounds. In addition to direct patient care, the physician will provide clinical oversight, mentorship, and collaboration for a team of nurse practitioners and physician assistants, ensuring high-quality, evidence-based care across multiple facilities. Primary Responsibilities Conduct routine patient rounds in skilled nursing facilities and assisted living communities. Evaluate and manage acute and chronic medical conditions. Perform history and physical examinations, admissions, follow-up visits, and discharge evaluations. Develop individualized treatment plans. Order and interpret laboratory studies, imaging, and diagnostic testing. Coordinate care with nursing staff, rehabilitation services, specialists, hospice, home health, and family members. Respond to urgent clinical issues within assigned facilities. Complete accurate and timely documentation in the electronic medical record. Participate in interdisciplinary care conferences and quality improvement initiatives. Maintain compliance with CMS, Medicare, state, and facility regulations. Leadership Responsibilities Provide clinical oversight for Nurse Practitioners and Physician Assistants. Review complex patient cases and assist with clinical decision-making. Serve as a physician resource for advanced practice providers. Promote evidence-based medicine and quality improvement initiatives. Foster collaborative relationships with facility administrators and nursing leadership. Assist in onboarding and mentoring new providers. Support organizational growth through physician leadership and clinical excellence. Qualifications Required MD or DO degree from an accredited medical school. Board Certified or Board Eligible in Internal Medicine or Family Medicine. Active California Medical License. Active DEA Registration. Current BLS certification. Excellent communication and leadership skills. Ability to work independently while supervising a multidisciplinary team. Reliable transportation for travel between facilities. Preferred Experience in Geriatrics, Hospital Medicine, Skilled Nursing, Long-Term Care, or Post-Acute Care. Experience supervising Advanced Practice Providers.
