As a Case Manager with our Community Support Program(CSP) and help adults living with serious and persistent mentalillness and co-occurring substance use disorders build stability, independence,and meaningful lives in their community.

This is an opportunity to do purpose-driven work whilebuilding your clinical career. You'll receive hands-on experience, workalongside an Assertive Community Treatment (ACT) team, and have access to clinicalsupervision toward licensure.

Starting Pay: $44,000

What You'll Do

As a CSP Case Manager, you'll help participants move fromwhere they are today toward where they want to be.

Your responsibilities will include:

• Completing ongoing assessments and developing person-centered treatment plans.
• Providing case management, supportive psychotherapy, symptom management, and crisis intervention within your scope of practice.
• Monitoring mental health symptoms and coordinating medication-related needs.
• Supporting participants in developing and achieving attainable recovery goals.
• Coordinating mental health, healthcare, and substance use treatment services.
• Helping participants access and maintain housing.
• Supporting money management and other activities of daily living.
• Helping participants pursue employment and educational goals.
• Assisting with appointments, transportation, scheduling, and community resources.
• Conducting frequent home visits and community-based outreach.
• Providing transportation to participants when needed, including driving an agency van in circumstances required by program policy.
• Working closely with participants' families and natural supports.
• Participating as an active member of an Assertive Community Treatment (ACT) team .
• Participating in group facilitation as assigned.
• Attending daily staffing, weekly team meetings, and in-service training.
• Maintaining accurate and timely clinical documentation in accordance with agency policy and DHS 63.
• Managing participant funds when appropriate.
• Collaborating with teammates to address both planned and unexpected participant needs.

This Isn't Your Typical Case Management Job

About 70% of your work happens in the community andparticipants' homes .

You may spend your day helping someone move into stablehousing, accompanying a participant to a medical appointment, connectingsomeone with substance use treatment, coaching someone through an importantlife skill, or helping de-escalate a crisis.

You'll work in real-world environments where progress isn'talways linear.

What You'll Bring

Education

• Bachelor's or Master's degree in behavioral science or a related field, such as:
  • Social Work
  • Psychology
  • Educational Psychology
  • Counseling
  • Marriage and Family Therapy
  • Related behavioral health discipline

Skills & Knowledge

• Knowledge of mental health and/or substance use disorders.
• Understanding of, or willingness to learn, the DSM-5 and psychotropic medications.
• Strong interpersonal and communication skills.
• Ability to respond appropriately during crises and utilize de-escalation techniques.
• Knowledge of motivational interviewing and trauma-informed care is preferred.
• Strong organization, time management, and documentation skills.
• Ability to manage multiple responsibilities in a fast-paced environment.
• Ability to maintain professional boundaries and exercise sound judgment.
• Ability to work effectively with a diverse population.

Other Requirements

• Valid driver's license.
• Reliable personal vehicle and sufficient insurance to meet agency requirements.
• Ability to pass the required caregiver background check.
• Ability to travel throughout Milwaukee County and work in participants' homes and community settings.

Schedule

First Shift | Generally 8:00 a.m.-4:00 p.m.

• Approximately 40 hours per week.
• Flexible schedule required based on participant and team needs.
• Some weekends and holidays.
• Rotating after-hours hotline coverage.
• Approximately 70% community-based.
• Frequent participant transportation and home visits.
• Some days may include up to approximately five hours of driving.

The position requires the ability to navigate stairs andperiodically lift/carry approximately 20 pounds to assist participants withvarious tasks. Reasonable accommodations may be made to enable individuals withdisabilities to perform essential functions.
