Family-Centered Treatment Specialist: Home & Community Care

Health Connect America seeks a Family Centered Treatment Specialist to provide therapeutic services to children, adolescents, and families within the FCT model. The role involves home- and community-based service delivery and collaboration with the Treatment Team to support outcomes.
