WK Bossier Shreveport, LA Nursing Full-Time, Day, 0800-1630 Posted 07/15/2026 Req # 10375 Responsibilities Coordinate and manage care throughout the patient’s acute inpatient illness; ensure continuity of services and appropriate utilization and management of resources. Demonstrate proactive leadership in the care management process, including planning, facilitating, advocating, educating, monitoring, evaluating and measuring outcomes for each patient population. Commit to teamwork, assume responsibility and accountability for own duties and performance. Participate in the health system’s quality improvement initiatives, implementing disease management processes, performance improvement, quality and outcome management. Qualifications Valid Louisiana nursing license (RN or LPN). Prefer at least 5 years of clinical experience in an acute care setting, preferably with management experience; utilization and/or case management experience desirable.
