Registered Nurse (RN) General Purpose The primary responsibility of this position is to supervise the day‑to‑day nursing activities of the facility during your tour of duty in accordance with federal, state, and local standards, guidelines, and regulations. Essential Duties Every effort has been made to identify the essential functions of this position. The omission of specific statements of duties does not exclude these from the position if the work is similar, related, or is an essential function of the position. Administrative Functions Assist the Director of Nursing Services in directing nursing activities in accordance with rules, regulations, and guidelines. Participate in developing, maintaining, and updating written policies and procedures governing the department. Ensure reference material (PDRs, regulations, professional standards) at nurses’ stations is current. Maintain an up‑to‑date Nursing Service Procedures Manual. Ensure nursing personnel comply with the manual. Make written and oral reports to the Director regarding department operations. Review and recommend revisions to policies, procedure manuals, and job descriptions. Coordinate nursing services with other resident services to ensure continuity of care. Ensure compliance with personnel job descriptions. Help implement the facility’s quality assurance program for the nursing service department. Participate in facility surveys conducted by authorized agencies. Review and update resident discharge plans as required. Plan nursing services for resident discharge and report as necessary. Collaborate with nursing staff and support personnel on department services, programs, and activities. Admit, transfer, and discharge residents as required. Complete accident/incident reports. Interpret policies and procedures for personnel, residents, visitors, and agencies. Perform administrative duties such as completing medical forms, reports, evaluations, and charting. Maintain confidentiality of user IDs and passwords, reporting any violations. Protect resident protected health information and report violations. Report suspected unauthorized attempts to access the facility’s information system. Committee Functions Attend required committee meetings (Infection Control, Policy Advisory, etc.). Prepare written and oral reports of nursing service programs for committees. Provide information to the Quality Assurance and Assessment Committee. Help develop agendas for Care Plan Committee meetings. Review discharge plans for Care Plan Committee and implement committee recommendations. Personnel Functions Ensure sufficient licensed practical and registered nurses and certified nursing assistants for the tour of duty. Develop work assignments and delegate administrative authority to Charge Nurses. Ensure nursing aides are trained and supervised by a licensed nurse. Make daily rounds to confirm staff compliance with standards. Monitor absenteeism and report problems to the Director. Enforce dress codes, policies, and procedures. Review and address staff complaints and grievances. Maintain positive relationships with inter‑department personnel. Create a warm, calm environment throughout the unit. Conduct regular shift meetings to identify and correct problems. Review workforce composition and provide recommendations. Evaluate employee performance and make staffing recommendations. Administer disciplinary action fairly and without discrimination. Report occupational exposures and incidents of fraud. Ensure computer workstations are logged off or protected as policy requires. Nursing Care Functions Inform staff of new admissions and room assignments. Prepare rooms for new residents and greet them upon admission. Assist physicians as needed and encourage timely documentation. Consult with residents, families, and physicians to plan care and treatment. Notify physicians and next of kin when a resident’s condition changes. Visit residents daily to assess physical and emotional status. Ensure direct nursing care is provided by qualified personnel. Review nurses’ notes for completeness and relevance. Provide direct nursing care including IVs, specimen collection, and vital signs. Admit, transfer, and discharge residents as necessary. Report problems to the Director and assist in corrective action. Keep the Director informed of resident status through written and oral reports. Assist the Charge Nurse in monitoring seriously ill residents. Authorize restraints per facility policy. Ensure medication protocols are followed and monitor medication passes. Inspect nursing areas daily for cleanliness and safety. Check residents who cannot call for help frequently. Assist in transportation of discharged residents and notify families of death. Staff Development Plan, conduct, and schedule in‑service training for nursing staff. Assist in developing annual in‑service training programs (OSHA, TB, HIPAA, etc.). Assist in planning clinical supervision for nurse aide trainees. Participate in continuing education and maintain licensure. Safety and Sanitation Assist the Safety Officer in developing safety standards. Identify tasks with exposure to blood or bodily fluids and ensure appropriate training. Ensure adequate personal protective equipment is available. Monitor compliance with safety regulations and equipment usage. Maintain clean and sanitary nursing service and resident care areas. Enforce hand hygiene procedures. Participate in fire safety and disaster preparedness drills. Support infection control programs and reporting hazardous conditions. Ensure use of safety equipment during resident transfers. Equipment and Supply Functions Recommend equipment and supply needs to the Director. Maintain adequate stock levels of medications, supplies, and equipment. Assist in preventive maintenance programs for equipment. Implement safe operation procedures for nursing equipment. Ensure only trained personnel operate equipment. Monitor supply usage to reduce waste. Maintain MSDSs for hazardous chemicals. Care Plan and Assessment Functions Develop preliminary and comprehensive nursing assessments. Create written care plans identifying problems, goals, and responsible professionals. Encourage resident participation in care plan review. Review care plans for appropriateness and update based on needs. Ensure staff refer to care plans before delivering care. Assist in scheduling and revising MDS, RAPs, and Triggers. Ensure treatments align with care plans and resident wishes. Budget and Planning Functions Recommend adjustments to nursing services to maintain quality care. Report suspected fraud related to billing or cost reports. Supervisory Requirements Assist with overall supervision and management of nursing staff. As an RN, you are delegated the administrative authority, responsibility, and accountability needed for assigned duties. Qualifications and Experience Education and/or Experience: Minimum of a Nursing Degree from an accredited college or university. Training in rehabilitative and restorative nursing practices. At least 2 years of experience in a long‑term care facility preferred. Language Skills Read, write, speak, and understand English. Read technical procedures, policy and procedure manuals. Present information and respond to questions from managers and employees. Mathematical Skills Apply fractions, percentages, ratios, and proportions to practical situations. Reasoning Ability Make independent decisions when required and solve practical problems. Tactfully deal with personnel, residents, families, visitors, government agencies, and the public. Demonstrate leadership and supervisory ability and cooperate with other personnel. Plan, organize, develop, implement, and interpret programs, goals, policies, and procedures. Provide care appropriate to residents’ age‑related needs. Work harmoniously within a team and demonstrate team concepts. Maintain knowledge of nursing and medical practices, laws, regulations, and guidelines. Demonstrate patience, tact, cheerfulness, enthusiasm, and ability to manage difficult residents. Seek and incorporate new methods and principles into nursing practice. Accurately convey information concerning a resident’s condition. Do not pose a direct threat to the health or safety of others. Certificates, Licenses, Registrations Current, unencumbered, active RN license in the state. Current CPR certification. Knowledge and experience with PCC preferred. Complete all required continuing education and licensing. Maintain good standing with the State Board of Nursing. Physical Demands Move intermittently throughout the workday. Speak and write in English clearly. Manage mental and emotional stress of the position. Use hearing and vision (or prosthetics) adequately to perform duties. Work independently with personal integrity and adaptability. Meet general health requirements, including medical and physical examinations. Handle residents who are ill, disabled, elderly, emotionally upset, or hostile. Push, pull, move, or lift at least 25 pounds up to 5 feet and carry that weight a minimum of 50 feet. Assist in evacuation during emergencies if necessary. Work Environment Office areas and nursing service areas (drug rooms, nurses’ stations, resident rooms). Frequent interruptions and interactions with residents, personnel, visitors, and agencies. Subject to hostile and emotional residents, families, and personnel. Communicate with medical staff and department directors. Work beyond normal hours, including weekends, holidays, and emergencies. Fires, falls, burns, odors, dust, disinfectants, tobacco smoke, infectious waste, and hazardous chemicals may be present. Maintain liaison with residents, families, and support departments for comprehensive resident planning.
