MDS Coordinator

Come join our team and start making a difference! Legend Healthcare and Rehabilitation - Greenville is looking for an MDS Coordinator. Hours: Full Time. Licensure: Texas RN or LVN License.

Responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS)/ PPS and development of the Interdisciplinary Plan of Care, as well as completion of LTCMI (Long Term Care Medicare Information), or LOC (Level of Care) forms for the Medicare payment process when applicable. These duties are to be performed in a timely manner consistent with CMS guidelines and Legend healthcare standards. This individual will establish and maintain positive relationships with all members of the health care team.

Qualifications:

• Current RN or LVN license in the state of Texas
• Electronic Health Record Experience- preferably PCC experience
• Experiencing with RAI process and MDS certification preferred
• Experience in a Skilled Nursing and Long Term Care facility preferred
• Experience as a MDS Coordinator preferred.
