About the Position

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience License: Must have license in hand Certifications: CPR, BLS Must-Have: HomeCare HomeBase (HCHB) proficiency within the last 6 months OASIS proficiency within the last 6 months Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures. Responsible for the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits and periodic office meetings are required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5-8. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Contractor will be assigned between 5-10 patients per day that equate to roughly 6-8 points. If there is a situation where the contractor is unable to meet their financial needs due to low patient census, please reach out to discuss floating to a nearby clinic to pick up hours. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Traveling up to 75 miles from the assigned care center is required. Mileage will be reimbursed at the IRS standard rate. Local candidates are accepted. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation impacting billing and follow-up patient care may result in a cancellation. All travelers must be proficient in HomeCare HomeBase EMR and have used it within the last 6 months. If the technology has not been used in the last 6 months, a retraining course must be completed during compliance. No contract hours are guaranteed. Weekly productivity expectations are 30 points, which equate to 40 hours per week. All overtime must be pre-approved. If not receiving enough patients each week, the opportunity to float to another nearby clinic will be available to pick up patients.
