Develop and implement personalized treatment plans. Document patient progress and maintain accurate records using Homecare Homebase. Participate in weekly case conferences to discuss patient progress. Provide routine visits, 30-day re-assessments, and coordinate discharge plans. Qualifications Home Health and Oasis experience required. Homecare Homebase documentation experience necessary; training can be provided for the first two weeks at a reduced rate.
