Physical Therapist - Long-Term Care Facility
Provide physical therapy services to residents in a long-term care facility. Assess, plan, and implement individualized treatment programs. Work with interdisciplinary team to maximize patient mobility, function, and independence. Document patient progress and maintain accurate records. Educate patients, families, and staff on therapy goals and techniques. Full-time, day shifts; ASAP start. Located in Vacaville, California.
