Sundale Senior Living is seeking an experienced Resident Care Director (must be an LVN/RN) to provide clinical leadership and oversight for resident care services across our Assisted Living and secured Memory Care community .

As the community's primary clinical leader, you will ensure residents receive appropriate, person-centered care consistent with their assessed needs, service plans, physician orders, company policies, and Texas Assisted Living regulations.

This is a visible, hands-on leadership role. You'll work closely with medication aides and caregivers while partnering with the Executive Director, Resident Care Coordinator, Memory Care Director, physicians, pharmacies, outside healthcare providers, residents, and families.

What You'll Do

Lead Resident Care & Clinical Operations

• Oversee admission, annual, and change-of-condition resident assessments.
• Ensure service plans remain accurate, current, and compliant.
• Participate in pre-admission assessments to determine whether prospective residents can be safely and appropriately served.
• Respond promptly to changes in condition and ensure appropriate clinical follow-up.
• Conduct regular resident rounds throughout Assisted Living and Memory Care.
• Coordinate care with physicians, pharmacies, home health, hospice, therapy, laboratories, and other healthcare providers.
• Communicate significant changes in condition or care needs to residents, families, and appropriate providers.

Oversee Medication Management & Resident Safety

• Maintain oversight of the community's medication management program.
• Ensure medication orders are accurately entered, implemented, and documented.
• Conduct medication audits, medication-pass observations, and competency evaluations.
• Address medication errors, missed medications, documentation concerns, and unsafe practices promptly.
• Review falls, incidents, wounds, hospitalizations, and other resident safety concerns.
• Identify patterns and ensure appropriate interventions and corrective actions are implemented.

Lead & Develop the Resident Care Team

• Coach, educate, and hold medication aides and caregivers accountable for resident care standards.
• Partner with the Resident Care Coordinator and Memory Care Director to ensure resident care needs are communicated and completed.
• Develop the Resident Care Coordinator's leadership of daily Assisted Living floor operations.
• Participate in recruiting, interviewing, orientation, training, competency validation, and performance management.
• Address performance concerns through coaching, corrective action, retraining, or competency review.

Maintain Compliance & Quality

• Maintain compliance with Texas HHSC Assisted Living requirements , including applicable provisions of 26 TAC Chapter 553 .
• Maintain survey readiness at all times.
• Conduct resident record, medication, documentation, care, and regulatory compliance audits.
• Ensure documentation is complete, accurate, timely, and ready for regulatory review.
• Assist with investigations, complaints, incidents, Plans of Correction, and regulatory follow-up.
• Participate in quality assurance and performance improvement activities.

Support Residents & Families

• Serve as a primary clinical resource for residents and families.
• Respond professionally and promptly to resident and family concerns.
• Participate in family meetings and care conferences.
• Communicate significant changes in condition, care needs, or service delivery.
• Build collaborative relationships while maintaining appropriate clinical and regulatory boundaries.
  
  

What We're Looking For

Ideal Candidate Experience

• Current Texas LVN or RN license in good standing.
• Minimum three years of leadership experience overseeing a resident care team in assisted living, memory care, long-term care, or a comparable senior living environment preferred.
• Experience leading resident care operations in a community of approximately 40 or more units strongly preferred.
• Demonstrated experience supervising, coaching, training, and holding medication aides and caregivers accountable.
• Hands-on experience with electronic medication administration records (eMAR) and electronic health records.
• Experience with August Health preferred.
• Working knowledge of Texas assisted living regulations and HHSC requirements.
• Strong understanding of medication management, resident assessment, change-of-condition management, documentation, incident response, and care planning.
• Ability to participate reliably in on-call responsibilities.
• Current CPR/First Aid certification preferred.
• Certified Dementia Practitioner or comparable dementia training preferred.
