This is an in-person home health position based in Mesa County, CO, offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care.What We're Looking For:Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)Valid driver's license, reliable transportation, and auto insuranceStrong assessment and critical thinking skillsAbility to work independently and manage a structured caseloadClear, professional communication with patients and team membersComfort with documentation and EMR systems (HCHB preferred)Home health experience preferred, but not requiredWhat You'll Do:Perform comprehensive patient assessmentsDevelop and update individualized plans of careProvide skilled nursing care and appropriate interventionsMonitor patient progress and report changes to the care teamEducate patients and families on medications, treatments, and care plansCoordinate care with LPNs, therapists, caregivers, and providersComplete, timely, and accurate documentation in EMRParticipate in on-call rotationWork Environment: Home health offers a different pace and structure than facility-based care.
