Community Transition Advocate for Independent Living
Thresholds
Thresholds is seeking a Transition Coordinator, CCMTP in Chicago to join the Transition Team under the Comprehensive Class Member Transition Program. The role supports safe, community-based transitions for Class Members from institutional settings with Thresholds services in place. Responsibilities include assessment, linkage to housing and services, and collaboration with the CCMTP interdisciplinary team.
