Position Details:

• Main Office location: Lawrence, MA
• Service Area: Lawrence territory.
• Job Type: Full Time
• Schedule: Monday - Friday
• Salary Range: $75,000 - $90,000

Responsibilities:

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by physicians.
• Conducts initial assessments of patients and families to determine home care needs.
• Performs thorough physical assessments and gathers comprehensive medical histories.
• Develops and revises individualized plans of care based on patient status and evolving needs.
• Collaborates closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively addresses inquiries and concerns from patients, caregivers, and medical teams.
• Provides counseling and support to patients and caregivers before, during, and after treatment.
• Supervises home health aides as required.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Preferably 2+ years of nursing experience, with a focus on home health and geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.
