Social Services Coordinator

The Social Services Coordinator is responsible for coordinating and providing psychosocial services to residents and their families in accordance with federal and Minnesota state regulations. This role supports resident well-being, discharge planning, care conferences, and advocacy while ensuring compliance with skilled nursing facility (SNF) requirements.

The Social Services Coordinator conducts psychosocial assessments and ongoing evaluations of residents' emotional, social, and environmental needs. They coordinate and participate in care plan meetings, family meetings, and interdisciplinary team discussions. They facilitate discharge planning, including coordination with community resources, hospitals, and outside agencies. They assist residents and families with adjustment to illness, disability, or long-term care placement. They advocate for resident rights and support grievance resolution. They provide counseling and crisis intervention as needed. They maintain accurate, timely, and compliant documentation in the medical record. They ensure compliance with federal, state, and local regulations, including CMS and Minnesota Department of Health requirements. They collaborate closely with nursing, therapy, admissions, and business office teams. They support advance directives, guardianship processes, and resident/family education.

The Social Services Coordinator requires a bachelor's degree in Social Work, Human Services, Psychology, or a related field. Minimum of 1 year of experience in a healthcare or long-term care setting is preferred. Knowledge of federal and Minnesota SNF regulations related to social services is required. Strong communication, documentation, and organizational skills are necessary. Ability to work effectively with residents, families, and interdisciplinary teams is essential.

Licensed Social Worker (LSW or higher) in Minnesota is preferred. Experience in skilled nursing or post-acute care settings is preferred. Familiarity with discharge planning and community-based resources is also preferred.

Ability to sit, stand, and walk throughout the facility. Occasional lifting of up to 25 pounds. Ability to manage multiple priorities in a fast-paced healthcare environment.
